New Participant Intake (90-Minute Session)
The New Participant Intake is a 1.5-hour, person-centered session designed to build a strong foundation for services. During this time, participants and, when appropriate, family members or support persons meet with an Inclusive Pathways team member to get to know one another, review program expectations, and begin developing an individualized understanding of needs, strengths, and goals. The session includes a review of background information, discussion of current supports and challenges, and completion of required intake documents. Participants are encouraged to share their preferences, communication styles, cultural considerations, and what success looks like to them. The intake concludes with an overview of next steps, service recommendations, and an opportunity to ask questions, ensuring participants feel informed, respected, and confident moving forward.
